Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Thank you and please contact our office if you have any questions at all.
For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed. Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.
Our dental team is dedicated to providing you with the best possible care. We will also assist you with receiving your maximum allowable insurance benefits. However, in order to do this, it's important to understand our financial policy. Dental insurance is a contract between you and your insurance company with specific benefits, percentage of coverage, deductibles and a yearly maximum. Knowing your plan will help you fully utilize your insurance and manage your financial obligations.
Our recommended treatment plan is never based on what your insurance will pay, but rather by what is clinically found. Our fees are based upon our professional time dedicated to providing the highest quality of care and our costs associated with that care. Our dental practice files patient insurance claims as a courtesy to our patients; however, all decisions are ultimately the patient's responsibility.
Your co-payment is to be paid the same day that your dental treatments is rendered. Your co-pay is based upon a percentage of the total amount due.
Please take time to review your insurance contract thoroughly and feel free to ask our staff for clarification on services, billings, and insurance questions or concerns.
